I have read and understand MSEA’s Human and Civil Rights Grant Policy and Guidance document.
I understand failure to comply with the grant policy and/or any associated documentation or reporting requirements will result in financial consequences for the individual/Local Association grantee and may potentially jeopardize any future grant funding being sought.
I understand that, if I am selected for this grant, I will need to submit an electronic W-9 form and complete MSEA’s Direct Deposit/ACH Authorization Agreement in order to receive grant funds.
Grant funds will only be paid to individual members or local associations. Grant funds are not paid to schools or school systems.
I am a current member of MSEA or a local association president (or authorized designee).
